Complete the online registration form below, noting which program you wish to attend. (FTE reserves the right to set qualifications to attend a program and select participants. See program details.)
Deposit ($100) must accompany application. FTE accepts Mastercard and Visa. Application is not considered complete until the deposit is received by the FTE and the applicant has receive a confirmation email. Note: If you choose to submit the deposit by check, please print the online application and send with the deposit to:

- Online applications received without deposit will NOT be considered.
- Faxed applications are NOT accepted.
Deposit Refund: After completing the program, or upon notification of unsuccessful application, deposits will be credited back to card or deposit check will be returned.
Cancellation Policy: Cancellation within 30 days of program start will result in forfeiture of deposit.

Important: If you cancel less than
30 days before the program you are scheduled to attend, you will forfeit
your deposit.
